Effective September 4th, all new guardians and conservators will be required to undergo a criminal history check, as announced by the Oakland County Probate Court.
The criminal history check form can be accessed at https://www.oakgov.com/courts/probate/Documents/in-house/Criminal%20History%20Check%20FormRE.pdf.
The form must be printed and signed by the nominee him/herself (not by a representative or attorney) and the search completed prior to the hearing date. To allow time for processing this form should be submitted with the petition.
Pursuant to MCR 5.302, the form will not be required for personal representatives at this time.
